State of Wisconsin Group Health Insurance Program

To enroll with Network Health, click on the Enroll Now button below. Follow the enrollment instructions and select Network Health as your health plan.

You can also download the Health Insurance Application/Change Form and complete it. Under Choose a Health Plan, write Network Health. Return the completed form to your employer.

In-Network Providers

Network Heath’s provider network is combined across northeast and southeast Wisconsin. We are only health plan in the Group Health Insurance Program with Froedtert Health, Children’s Wisconsin and Ascension Wisconsin all in-network. With SilverCloud you can access digital cognitive behavioral therapy solutions to support your treatment of behavioral and mental health conditions, including the following.

Using SilverCloud helps you take control of your mental health. To access SilverCloud, log in to your member portal, select the My Wellness link in the top right of the page and then select SilverCloud from the dropdown menu.



Available Pilot Programs for 2022

Network Health is always looking for ways to improve the health of our members. For State of Wisconsin members, we offer several pilot programs aimed at specific conditions.

  • Acupuncture Services for members with chronic headaches, neck pain, low back pain and osteoarthritis of the knee.
  • Prevent Type 2 Diabetes Program for members with pre-diabetes
  • Evidenced Based Integrated Care Plan (EBICP) Program offers enhanced dental benefits to members who are diabetic or are experiencing a high-risk pregnancy.

These pilot programs are not part of uniform benefits and are subject to change without notice.

Health Management Services

Care Management

Care management provides personalized coordination of care, access to care, identification of benefits and resources, and condition monitoring to help you get the health care you need, whether you’re dealing with a short-term illness or a more complex diagnosis.

Care management begins with a one-on-one conversation with a care manager. If you choose to participate, you will work directly with a care manager to manage your health condition with the ability to opt out at any time by contacting the customer service number on your ID card. Care management is available at no cost to members.

Condition Management

Our condition management team empowers you to take charge of your chronic condition and be the healthiest you can be. Our skilled team of registered nurses engages members in one-on-one health coaching about their ongoing care, provides educational resources and helps build connections to community programs. Condition management is available at no cost to members, and enrollment in the program is voluntary. Individuals who may benefit from condition management services include those who have:

  • Asthma
  • Chronic obstructive pulmonary disease (COPD)
  • Diabetes
  • Heart Disease
  • Stroke
